Must re-enroll every year.  Must certifiy that eligibility requirements are met on an annual basis.

Anthem Dental, EPIC Benefits+, Dental Wisconsin

Plans No Longer Available in 2019**

  • Supplemental dental coverage available through Delta Dental PPO
  • Supplemental vision coverage available through VSP
  • AD&D coverage available through Zurich AD&D
  • No option for hospital and surgical indemnity coverage for 2019
