Family Medical Leave Act (FMLA) Toolkit

Employees are generally eligible for medical leave under federal and Wisconsin law. When approved, employees are permitted to use all leave types based upon the parameters established for duration of leave based on the unique medical situation.

Managers and supervisors should contact their local HR office for assistance in proper completion of the FMLA process.

Employees of Region 1 (DOT, DFI, DSPS, PSC, HEAB and DOA and the attached agencies) should use the Region 1 FMLA Application for new 2025 requests through November 16, 2025. Recertification for 2026 should be completed in the Medical Leave and FMLA Request Application.

All other employees should use the Medical Leave and FMLA Request Application to submit their request.
